This is the climate change questionnaire questions in the survey I answered in early June 2012 and commented upon here.
UPDATE
It has been brought to my attention that this is NOT the questionnaire used in Lewandowsky et al 2012. There are similarities but this appears to be a development of the 2010 survey, and was probably being trailed at Watching the Deniers site. It was located on the University of Western Australia site – try the link. THE ACTUAL (& shorter) QUESTIONNAIRE is analyzed here.
I believe that this survey is the basis of the recent paper:-
Lewandowsky, Oberauer & Gignac – NASA faked the moon landing|Therefore (Climate) Science is a Hoax: An Anatomy of the Motivated Rejection of Science (in press, Psychological Science)
Note to Professor Lewandowsky of University of Western Australia
If this is not the original survey, but has been doctored in any way, I will happily publish the actual survey. I will also provide details of the researcher and the url for any investigation. I can be contacted through the moderated comments. I am not aware of any copyright restrictions on reposting the questions. I accessed this from “Watching the Deniers” website, where there was no mention of copyright material. Neither was there any mention of copyright on the introductory front page. The doing a search I only came across a link to a 2010 survey. Neither could I find a link within The University of Western Australia Website, though it is on their servers.

/ 30/07/2012So, what’s the point? What were the results of the “survey”? It seems designed to look for political/religious/scientific attitudes, probably subject to biased interpretation, but your post gives little info about the outcomes.

/ 30/07/2012Please see by later “Lewandowsky et al. 2012” postings and the original paper. Also my analysis of the questionnaire in June.

/ 30/08/2012This is not the exactly the same survey as used in the “Lewandowsky et al. 2012″ paper, although some of the questions are the same.
For example, the paper (see table 2 at the end) has a question about Pearl Harbor that is not on your list. Also there was a question about acid rain not being a serious threat.
I think the survey you have here is a later modification.
Follow Jo Nova’s blog for more to come on this story.

/ 30/08/2012Thanks for that Blackadder. Do you know where I might obtain a copy of the original questionnaire? I can see now that the survey I took was a later modification. However, there are more similarities than differences.
I have now the raw data on the Lewandowsky paper and will post both here and Jo Nova’s blog in the coming days.

/ 01/09/2012You are correct. I will post up a correction.
The comparison should be with the results of the four questions on beliefs in the Climate Science to be found on Page 27 of the paper. I have been working on this comparison and hope to post up something in the next 24 hours.
